Job Summary and Qualifications

POSITION SUMMARY: Responsible for developing and coordinating initiatives directed at improving and assuring the safe use of medications within the organization. This includes developing and maintaining productive working relationships and providing support to other disciplines on which safe medication use is dependent, standardizing medication management practices, ensuring regulatory compliance, implementing national quality agenda related to medication management, medication safety programs, and staff development/education. Medication Use Safety

1. Develops a strategic plan to improve the safety of the medication-use system.

2. Assesses the medication management system for risk points and identifies strategies to improve safety.

3. Conducts at least one FMEA annually.

4. Coordinates the reporting and analysis of adverse drug reactions.

5. Coordinates the reporting and analysis of medication errors.

6. Develops policy and procedures that promote safe medication use practices.

7. Participates in the development, implementation, and maintenance of medication-use safety initiatives.

8. Effectively uses the appropriate analytic tools and process improvement techniques to assess andimprove medication-use processes.

9. Coordinates with organizational leaders to assure that medication-use processes are conducted in asafe physical environment.

10. Coordinates the development and implementation of strategies to eliminate or minimize medication errors.

11. Assesses and reports medication use safety improvement activities to leadership and appropriate committees.

12. Ensures appropriate and safe use of medication-related automation including smart pumps, order entry systems, and automated dispensing machines.

13. Assesses published literature to identify best practices and/or guidelines relevant to medication-use safety and opportunities for reducing medication errors. Performance Improvement

1. Chairs Drug Safety Team and submits report of medication safety activities to the Pharmacy and Therapeutics Committee

2. Chairs Medication Error Team and submits report of medication safety activities to the Drug Safety Team, Patient Safety Team, and Pharmacy and Therapeutics Committee

3. Maintains a performance improvement program that improves medication safety throughout the  organization. Regulatory Compliance

1. Complies with federal, state, and local regulations and accreditation standards relating to medication- use safety.

2. Assesses and reports compliance with Medication Management accreditation standards.

3. Develops policy and procedures to meet regulatory compliance standards.

4. Reviews controlled substance waste and return records and reconciles discrepancies.

5. Assesses and documents compliance with corporate controlled substance monitoring requirements.

6. Monitors for controlled substance theft or diversion, assists with investigations and reporting requirements.

Education and Training

1. Manages pharmacy staff development, competency and training in collaboration with the Pharmacy Clinical Coordinator and Pharmacy Supervisors.

2. Provides medication-use safety education to pharmacy staff, healthcare providers and other appropriate staff.

3. Provides orientation and training to new staff members.

4. Provides regulatory or accreditation compliance education to healthcare providers and other appropriate staff.

5. Provides education and training to pharmacy students.
